Coastal Care Partners

Parkinson's Disease Home Care

Parkinson's is progressive and unpredictable day-to-day. Symptoms fluctuate—mobility, energy, and medication effectiveness can shift within hours. Families need caregivers who understand these rhythms and a clinical team that coordinates care as the disease evolves.

Coastal Care Partners delivers nurse-led Parkinson's home care. We emphasize safety, mobility support, routine, and medication complexity. Our CCU-trained caregivers work alongside RN Care Managers who update care plans, coordinate with neurologists, and ensure your loved one receives the structured support that keeps them safe at home.

Parkinson's home care—caregiver providing mobility support

Why Parkinson's Care Requires Specialized Support

Parkinson's introduces challenges that standard companion care cannot address. Gait changes, freezing episodes, and fall risk require caregivers trained in mobility and transfer safety. Mobility changes and freezing episodes often require coordinated therapy support—care that works alongside physical therapy, not in isolation. Medication timing is critical—many individuals experience "on" and "off" periods that affect function throughout the day.

Sleep disruption, fatigue, and caregiver strain accumulate over time. Families need a team that understands the condition and can adapt care as symptoms change.

  • Gait changes, freezing, fall risk — Mobility support and transfer techniques matter for safety.
  • Tremor and rigidity — Daily tasks require patience and adapted approaches.
  • Fatigue — Energy fluctuates; routines should accommodate good and difficult days.
  • Swallowing and nutrition — Some individuals benefit from modified textures and pacing at meals.
  • Medication timing sensitivity — "On/off" periods require coordinated care and observation.
  • Sleep disruption and caregiver strain — Families need relief and a team that shares the load.

Our Parkinson's Care Model

We collaborate with physical therapists experienced in neurological movement disorders. Therapy can support gait stability, transfer safety, strength, and fall prevention. Our RN Care Managers coordinate with PT providers to align therapy recommendations with in-home care plans—so caregivers reinforce what therapists prescribe and care stays consistent.

CCU-Trained Caregivers

Our caregivers complete Coastal Care University training, including mobility and transfer safety. They are prepared for gait support, freezing episodes, and the pacing that Parkinson's requires—not just companionship.

RN Care Manager Oversight

A dedicated RN Care Manager coordinates care plans, updates them as symptoms evolve, and communicates with neurologists and specialists. Your quarterback for Parkinson's care at home.

Learn about care management

Medication Organization + Physician Coordination

Medication timing matters. We help organize schedules, coordinate with physicians on adjustments, and ensure caregivers understand when to observe for "on" and "off" periods.

PT/OT Coordination & Home Safety

We coordinate with physical and occupational therapists, reinforce exercise programs, and support home safety modifications that reduce fall risk and support independence.

Escalation When Symptoms Change

When symptoms change, we document, assess, and communicate with physicians. Our clinical team can evaluate and escalate when higher-level support is needed.

Clinical oversight

When Higher-Level Medical Support Is Needed

As Parkinson's progresses, some individuals require in-home nursing support—medication management, clinical monitoring, or coordination with specialists. Our integrated model delivers acute-level support at home when appropriate.

Our RN Care Managers and clinical team work with neurologists and primary care physicians to keep care stable—monitoring changes, reconciling medications, and addressing acute issues before they escalate. We are not a replacement for emergency services. We are a structured system designed to provide advanced, high-acuity support at home when it is clinically appropriate.

Serving Families Across Coastal Georgia & South Carolina

We provide Parkinson's home care to families in Savannah, Bluffton, Saint Simons Island, and surrounding coastal communities. Our nurse-led model is the same across our service area—CCU-trained caregivers, RN Care Manager oversight, and clinical integration wherever your loved one calls home.
